Chi Partners worked with the Medicaid Long-Term Care Division of the Colorado Department of Health Care Policy and Financing to develop a strategic plan focusing on proposed changes to the Medicaid long-term care environment in Colorado.
These changes entailed significant revisions to Colorado’s long-term care programs in an effort to deliver services more efficiently, improve the quality of care provided, and continue to deliver a robust package of services within a tight budget environment.
Chi Partners assisted the LTC Division in creating a “road map” with clear goals, objectives and timelines to effectively navigate these changes. This work included a review of the Division’s mission, vision, and principles; internal and external environmental scans; goals and objectives; tactical plans including benchmarks, timelines, resources needed and expected outcomes; a strategy grid by which the Division could gauge progress and initiate course changes if needed; and a final written strategic plan.
